Stay Steady is a self-referral service either via email or phone, we do not accept clinician referrals.

This is to ensure the patient is engaged with the service we offer. We offer phone consultation which may be followed up by a face to face consultation if required. We aim to identify potential falls risks and work with the client to reduce these. Suitable patients are normally mobile. able to access their community and do not have care needs. We do not offer home assessments/visits.

RCT refer people to their GP to access the frailty team. However we can refer people into the Don’t Fall For It scheme.

Aneurin Bevan 

Gwent Frailty is a multi-disciplinary service within the Primary Care and Community Services Division in Aneurin Bevan University Health Board and the five Gwent Local Authority Social Care Services, centred on providing patients with care and/or treatment closer to home and promoting patient independence.
